IHCS of Indiana Terms of Service

Integrative Health Care Services of Indiana (IHCS of Indiana) offers SMS messaging services to enhance communication between clients and their care coordinators. By enrolling in or using our SMS services, you agree to the terms outlined below:

1. Message Frequency
   Clients may receive up to 30 SMS messages per month. These messages are intended to facilitate communication with your care coordinator, who will check in to determine if further contact by phone is needed.

2. Cost and Fees
   Messaging and data rates may apply, depending on your mobile carrier and plan. Please consult with your carrier for details. IHCS of Indiana is not responsible for any fees incurred.

3. Opting Out
   You may opt out of receiving SMS messages at any time by:

   • Dialing (317) 854-6306
   • Contacting us via email at info@ihcsindiana.com
   • Filling out the form above and selecting “Opt-Out”

   Once we receive your opt-out request, we will remove your number from our SMS list. Please allow up to 3 business days for processing.

4. Privacy and Data Use
   IHCS of Indiana values your privacy. Your information will only be used for the purposes of facilitating communication with your care coordinator and will not be shared with third parties without your consent, unless required by law.

5. Changes to Terms of Service
   IHCS of Indiana reserves the right to modify these terms at any time. Changes will be communicated via email or SMS, and continued use of our SMS services constitutes acceptance of the revised terms.
